Online Membership Form
Gym Membership (4 options)
- Complete member details & health questionnaire
- Choose desired gym membership in the drop-down menu (4 memberships to choose from). 3 new Windows will open
- Choose a start date (we can modify this if need be)
- Leave tag number vacant (issued with gym induction brief)
- Leave "admin fees" and "direct debit arrangement" information
- Choose your preferred method of payment under "ongoing payment" and complete. If after your FREE week you decide not to continue training at the "Happiest Gym on Earth", your account WILL NOT be debited
- Manually sign using your mouse/finger and click "confirm signature" (does not need to be perfect). 2nd box is only if a 3rd party is paying your membership fees or if you are 16 or 17 yo a parent/caregiver must sign
- Fill out all drivers license details under "electronic signature"
- Tick the terms and conditions (these will be e-mailed to you) and click submit
With all new gym memberships, payment schedules will not commence until 7 days after members have received their access key and had the Gym induction brief. The online membership form simply saves time and cuts the signup procedure time in half. Debit First will charge your nominated account fortnightly, 6 monthly or annually depending on your choice of membership (see bottom of page for fee structure).
Group Training Membership
- Complete member details & health questionnaire
- Choose your preferred group training membership in membership section (3 new Windows will open)
- Choose a start date you intend to commence group training
- Leave tag number vacant
- Leave "admin fees" and "direct debit arrangement" information
- Choose your preferred method of payment under "ongoing payment" and complete. If after your FREE week you decide not to continue training at the "Happiest Gym on Earth", your account WILL NOT be debited

Fitness Passport Members
All fitness passport members are to fill out the online membership form as per the instructions above. When entering type of membership, select number 4 fitness passport in the drop down menu.
There is no requirement to fill out your bank details. When you hit submit at the bottom of the form a pop-up box will show up saying you need to fill out banking details. Simply click the continue without bank details option.
Please bring your fitness passport membership card with you to the gym induction once you are booked in.
NOTE** Please enter your fitness passport member number into the appropriate field in the online membership form.
Fee Structure: All member debits are processed on Tuesdays. Depending on your membership debits may occur weekly, fortnightly, 6 monthly, or annually. Our debit company provider, Debit First charge a $0.50 transaction fee to all Direct Debit members per transaction. All Credit Card payments are debited to current credit card % fee rates; AMEX is a higher rate. In the event that there are insufficient funds or incorrect banking/credit card information, Debit First will impose a $6.60 default fee to your next scheduled payment. If you have 2 consecutive failed payments, your membership will be automatically cancelled. Those on the six-month or 12 month gym memberships will need to organise a manual payment in the event that their initial payment does not clear.
Gym members – If you wish to pause or suspend your membership for a certain time frame, there is a $33 reactivation fee on your return. Any FIFO employees that wish to join the gym, special arrangements will be considered.
